Improvement with spironolactone is expected in 4-6 weeks. Spironolactone is used OFF label to treat acne. This means that your doctors should consider that versus the other FDA cleared alternatives. Spironolactone is contraindicated in patients with renal insufficiency, pregnancy, abnormal uterine bleeding, estrogen-dependent malignancy, and in combination with drugs that could lead to hyperkalemia. Treatment with birth control pills or spironolactone can be considered in late-onset (>25) or persistent acne, acne that flares around the menstrual cycle, acne that is more severe on the lower face or neck or resistance to conventional therapies. 🙂
